HIPAA Notice of Privacy Practices
Effective Date: July 25th, 2024
The following policy describes how your personal and
medical information could be used and disclosed. This
policy also describes and explains your rights and how
you may obtain access to this information. Please take
some time to review this document. We are required by law to maintain your
confidentiality and honor HIPAA.
Always Reddy Nutrition Pledge regarding Protected Health Information:
I understand that protected health information about you and your health is very
personal. We will always be committed to protect your health information. This
notice will give you information about how your PHI may be used. The law requires
me to explain how, when, and why I use and disclose protected health information,
notify you on how I take precautions to protect your PHI about you, and make sure
that your protected health information is kept private and secure.

You can also send a complaint to the Secretary of the United States Department of
Human Services within 180 days of the complaint. If you file a complaint, I will not
take any action against you or change the treatment plan of your care in any way.

Your Rights Regarding any Protected Health Information (PHI)
Below lists the following rights you as the client have regarding PHI we maintain
about you:

  1. Right to inspect and copy: You have the right to inspect and copy PHI that
    may be used to make decisions about your care. This typically includes medical
    and billing records. If you would like a copy of this information we may charge
    you a fee for the costs of copying, mailing, or other supplies associated with
    your request. We will respond to your request within 30 days.
  2. Right to amend: At any point if you feel like the PHI is incorrect or
    incomplete, you may request to amend or supplement the information. To
    request an amendment your request needs to be written and submitted to
    Allison P. Reddy MS, RDN, LDN. We may deny your request for an
    amendment if it is not in writing or if it does not include a reason to support
    the request. If the amendment is denied we will provide a written denial to
    you as soon as the decision has been made.
  3. Right to an Accounting of Disclosures: You have the right to request
    “accounting of disclosures”. This is a detailed list of the disclosures we made of
    PHI about you. To request this list you must submit your request in writing to
    Allison P. Reddy MS, RDN, LDN. You may ask for disclosures at any point in
    time. The list of disclosures we provide does not have to include the following:
    billing and collection information about payment for treatment, health care
    operations, occurring as a byproduct of permitted use and disclosures, for
    national security or intelligence purposes or to correctional institutions or law
    enforcement regarding inmates, and as part of a limited data set of
    information that does not include any information identifying you.
  4. Right to Request Restrictions and Confidential
    Communications: You have the right to ask for a restriction or limitation on
    the PHI that we use or disclose about you for treatment, payment, or health
    care operations or to persons involved in your care. We are not required to
    honor your restriction request. You have the right to request for us to
    communicate with you about medical concerns in a certain way or location. To
    request restrictions and confidential communications, you must make your
    request in writing to Allison P. Reddy MS, RDN, LDN.
  5. Right to a Paper Copy of this Notice: You have the right to request a
    physical paper copy of the following private policies and privacy practices.
    Uses and Disclosures of Protected Health Information (PHI)
    The work we do at Always Reddy Nutrition will sometimes require us to use your
    PHI. Below are different ways that we can use and disclose protected health
    information without your written authorization.
  6. For Treatment: We may use PHI about you to provide you with, coordinate
    or manage your medical treatment or services. We may disclose information to
    a variety of allied health professionals like doctors, nurses, medical students,
    etc. We may also disclose PHI to providers outside of Always Reddy Nutrition
    who may be involved with your medical care and services. We also may use
    PHI to contact you as a reminder of appointments for treatment at Always
    Reddy Nutrition. We may also use and disclose PHI to tell you about or
    recommend possible treatment options or services that may be of interest to
    you.
  7. For Payment Services: We may use and disclose your PHI so that we can bill
    insurance and receive payment from an insurance company or 3rd party.
    Sometimes we need to give your health plan information about nutrition
    services you receive at Always Reddy Nutrition so that your health plan will
    reimburse you for nutrition services you receive. We may also tell your health
    plan about the nutrition services you are going to receive to obtain prior
    approval or to determine if your health plan will cover the treatment.
  8. For Health Care Operations: We may use and disclose PHI about you for
    Always Reddy Nutrition health care operations, such as quality assessment
    and improvement activities, case management, coordination of care, business
    planning, and customer services.
  9. As Required by Law: We will disclose PHI when required to do so in
    accordance with state, local, and federal laws.
  10. Health Risks: We may disclose PHI about you to a government authority if
    we reasonably believe that you are a victim of abuse, neglect or domestic
    violence. We will only disclose this type of information to the extent required
    by law.
  11. Health Oversight Activities: We may disclose PHI to a health oversight
    committee/agency if they request due to audits, investigations/inspections, as
    necessary for licensure and for the government to monitor a health care
    system.
  12. Law Enforcement: We may release PHI as required by law or in response to an
    order or a warrant of a court, a subpoena, or administrative request. We may
    also disclose as needed if law enforcement requests as well.
  13. Workers Compensation: We may disclose your PHI as necessary to comply
    with laws regarding workers compensation or other similar programs.
  14. Judicial & Administrative Proceedings: We may disclose your PHI in
    response to a court or administrative order, if you are involved in a lawsuit or
    dispute. We may also disclose PHI about you in response to a subpoena,
    discovery request or other lawful process by someone else involved in the
    dispute. This only occurs if efforts have been made, either by us or the
    requesting party, to obtain an order protecting the PHI requested.
    10.Business Associates: we may disclose PHI to business associates who perform
    services like billing companies but they must also safeguard your PHI.
    11.Public Health: We may disclose your PHI to a public health or legal
    authorities in charge of preventing or controlling disease, injury, or disability
    as required by law.
    12.Serious Threat to Health or Safety: If a serious threat to your health and
    safety or the health and safety of the public or another presents we may use
    and disclose your PHI when necessary.
    You Can Object to Certain Uses and Disclosures
    Unless you object or request that only a limited amount of information/PHI be
    shared, we may disclose PHI about you in the following conditions:
  15. We can share information with a family member, friend, or other person
    identified by you that is directly relevant to that person’s involvement in your
    care or payment for your care. We may also share PHI to notify these people of
    your location, general condition, or death.
  16. We may share PHI with a public or private agency (aka FEEMA) for disaster
    relief purposes.
